The role of a low dose of propofol before extubation in children with upper respiratory tract infection: a prospective randomized controlled trial

The administration of a low dose of propofol was previously used in children to reduce the incidence of bronchospasm and laryngospasm. However, it was never used in children with upper respiratory tract infections. The aim of this study was to determine the effect of a low dose propofol on the respiratory complications immediately after tracheal extubation in children undergoing abdominal surgery.

Matériel et méthodes

This randomized double blinded clinical trial that was registered in the Pan African Clinical Trial Registry (PACTR202412599188741). 144 children, 1-6 years old with ASA I-II classes, undergoing abdominal surgeries were involved. The anaesthetic and surgical management was standardised. Patients were randomly divided into two groups: group 1 received 0.5 mg propofol/kg one minute before awake extubation, and group 2 received normal saline. Finally, the presence of post-extubation respiratory adverse events was assessed. The significance level was set at p<0.05, and the relative risk was given with a 95% confidence interval.

Résultats & Discussion

Demographic parameters and per operative respiratory monitoring data were comparable between both groups. The low dose of propofol reduced the risk of tachycardia (p=0.0001; RR= 0.290 [0.146-0.577]), respiratory adverse events (p=0.003; RR= 0.393 [0.178-0.867]), and delirium emergence (p=0.0001; RR= 0.074 [0.011-0.507]) following tracheal extubation.  It also reduced the need for oxygen support in the PACU (p=0.0001; RR= 0.074 [0.011-0.507]).

Conclusion

The use of a low dose of propofol one minute before awake extubation seems to be safe and beneficial in children undergoing abdominal surgery while suffering from upper respiratory tract infection.
